Frederick W. Polak, 98, of Warren, died Wednesday, Nov. 21, 2018 at the Rhode Island Veterans' Home in Bristol. He was the husband of the late Emily Susan (Fortuna) Polak.
Frederick was born in Warren, a son of the late Simon and Johanna (Tupaj) Polak. He was an insurance salesman for many years for the Prudential Insurance Co.
He was a Veteran of the US Army and the US Air Corps, serving as a Pilot in WWII and Korea, with the rank of First Lieutenant. He received the Distinguished Flying Cross for his service.
Mr. Polak was a lifelong Warren resident and parishioner of St. Thomas the Apostle Church.
He is survived by his sons, Steven M. Polak of Dallas, Texas, Matthew J. Polak of Warren, and Thomas and his wife Kathryn(Gianello) Polak of Hudson, N.H.; and his daughter, Kathryn A. Polak of Canton, Ma.
A Mass of Christian burial was held Monday, Nov. 26, 2018 in St. Thomas the Apostle Church. Burial was in St. Jean the Baptiste Cemetery, Warren.
Arrangements by Smith Funeral and Memorial Services, 8 Schoolhouse Road, Warren.
