Each week the Personal Injury Law Offices of d’Oliviera & Associates compiles a summary of recalls, FDA warnings, market withdrawals and safety alerts that they feel the public should be aware of for their safety and so that they can make informed decisions.

Hre are 14 recalls to be aware of this week.

Medical Devices & Equipment

• Defibrillation Electrodes for Lifepak AEDs by Physio-Control: Class I Recall – Incorrect Placement Instructions for Infants Depicted on Artwork.

Pharmaceuticals/Drugs

• AuroMedics Pharma LLC Issues Voluntary Nationwide Recall of Pantoprazole Sodium for Injection 40 Mg Per Vial, Due to Presence of Glass Particles in the Vial.

Auto

• Mazda North American Operations is recalling 1,076 model year 2017 CX-5sequipped with an accessory trailer hitch wiring harness. This harness may be routed such that there is insufficient clearance between the harness and the exhaust muffler, allowing the wiring harness to contact the muffler.
• Mazda North American Operations is recalling 144 model year 2018 Mazda3s equipped with an incorrect exhaust valve spring. The incorrect exhaust valve spring may fail while the vehicle is being driven, potentially causing an engine stall.

• Kia Motors America is recalling 2,014 model year 2018 Sorentos. The side curtain air bags on these vehicles have a component that may detach during deployment of the inflatable curtain air bag. If the component detaches during deployment, it may enter the passenger compartment and be a projectile within the vehicle cabin, increasing the risk of injury.
• Mazda North American Operations is superseding a previous recall for 19,781 model year 2004-2006 B-series trucks. Upon deployment of the passenger side front airbag, excessive internal pressure may cause the inflator to rupture with metal fragments striking the vehicle occupants, potentially resulting in serious injury or death.
• Fiat Chrysler recalls 1.8 million Ram pickups that can shift out of park. Gear shifters in 1.8 million Fiat Chrysler pickup trucks could accidentally switch out of park, triggering a major recall.

Utility/Recreational Vehicles & Automotive Accessories

• Grand Design RV is recalling 114 model year 2018 Reflection travel trailers, model 297RSTS. The vehicles may have insufficient clearance between the tire and the ram that moves the slide out room. The tire can contact the ram for the slide out room while the trailer is being towed, causing tire damage or a possible tire blowout.
• Travel Lite RV is recalling 163 model year 2018 Falcon recreational trailers, models F-23TH, and F-23RB. A 2″ Class III hitch coupler was mistakenly installed instead of a 2 5/16″ Class III hitch coupler. Because the coupler is under rated for the gross vehicle weight rating (GVWR) of the trailer, the trailer could become unhitched from the tow vehicle, increasing the risk of a crash.
Recalled Recreational Vehicles
• Polaris Recalls RZR XP 4 Turbo Recreational Off-Highway Vehicles Due to Fire Hazard. The return fuel line can be improperly secured which can cause fuel to leak, posing a fire hazard.
• John Deere Recalls Crossover Gator Utility Vehicles Due to Crash Hazard. The steering shaft can separate from the steering rack assembly and result in a loss of vehicle steering control, posing a crash hazard.

Food, Beverage & Nutritional Supplements

Recalled Apples
• Jack Brown Produce, Inc. Recalls Gala, Fuji, Honeycrisp and Golden Delicious Apples Due to Possible Health Risk.
• Meijer Voluntarily Recalls Select Meijer Brand Fresh Packaged Products Containing ApplesDue to Potential Health Risk.
• Fresh Pak Inc. Recalls Lot Specific Sliced Apple Products Because of Possible Health Risk.
• PFP Enterprises LLC is recalling approximately 7,570 pounds of ground beef products due to misbranding and an undeclared allergen, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The products contain soy, a known allergen, which is not declared on the product label.
• Gel Spice Company, Inc. Issues Allergy Alert on Undeclared Soy and Wheat in Beef Flavored Bouillon Cube Products.
Recalled Smoked Salmon
• Springfield Smoked Fish Expands their Recall of Smoked Salmon Because of Possible Health Risk.
• Nodine’s Smokehouse Inc. is Recalling Smoked Salmon 1.5 lbs, 8 oz Packages Due to Possible Listeria Monocytogenes Contamination.
• Voluntary Recall on Harris Teeter’s HT Traders Mama Biscotti Mini Biscotti – Triple Chocolate.
• Marmex Corp. of Orange, Calif., is recalling all lots of 500mg Blue Pearl All Natural Male Enhancement Supplement. The product contains sildenafil as an active ingredient which is not declared on the label, making it an unapproved drug for which safety and efficacy have not been established.

Infant & Children Products

Recalled Children Sleepwear
• Wohali Outdoors Recalls Children’s Sleepwear Due to Violation of Federal Flammability Standard; Sold Exclusively at Bass Pro Shops. The children’s pajama sets fail to meet flammability standards for children’s sleepwear, posing a risk of burn injuries to children.
• Squatty Potty Recalls Children’s Toilet Step Stools Due to Injury and Fall Hazards. The top removable step (hat) on the recalled children’s toilet step stools can detach while a child is standing on it, posing injury and fall hazards.

Sporting Equipment

• Brompton Bicycle Recalls Bicycles Due To Fall Hazard. The bicycle’s bottom bracket can break during use, posing a fall hazard.

Household Products

• Vilo Home Recalls Marseille Dining Benches Due to Fall Hazard. The foam seat cushion and base can crack in the middle and cause the bench to collapse, posing a fall hazard.
• Crate and Barrel of Northbrook, Ill., is recalling about 7,400 Dakota benches that can tip over while in use, posing a fall hazard.
• Monte Carlo Recalls Ceiling Fans Due to Injury Hazard. The brackets connected to the fan blades can break, causing the blades to fall, posing an injury hazard.

• Hunter Fan Company of Memphis, Tenn., is recalling about 170,000 Hunter Contempo ceiling fans sold in the U.S. and Canada.
• Superior Importers Recalls Mattresses Due to Violation of Federal Mattress Flammability Standard; Sold Exclusively at Amazon.com. The mattresses fail to meet the mandatory federal flammability standard for mattresses, posing a fire hazard.

Industrial/Tools

American Honda Recalls Portable Generators Due to Fire and Burn Hazards. The carburetor can leak gasoline, posing fire and burn hazards.

Apparel

• ACE Work Boots Recalled by Shoes for Crews Due to Injury Hazard. The work boots were advertised as puncture-resistant, but were not manufactured with the puncture-resistant plate, posing an injury hazard to users.
• Todd Snyder recalls Champion brand sweatshirts. The garments fail to meet federal flammability standards.

Miscellaneous

• Melville Direct Recalls Fiber Optic Christmas Trees Due to Fire Hazard. The AC power adapter can overheat, posing a fire hazard.
Recalled Fireworks
• Matrix Fireworks Recalls Fireworks Due to Violation of Federal Standard; Explosion and Burn Hazards. The recalled fireworks are overloaded with pyrotechnics intended to produce an audible effect, violating the federal regulatory standard for this product. Overloaded fireworks can result in a greater than expected explosion, posing burn and explosion hazards to consumers. The product is labeled as a fountain but functions similar to an M-80 or other large firecracker, which are illegal.
